Common Disaster Clause |
Select this check box if distribution of the proceeds of this employee's life insurance plan is governed by the Common Disaster Clause. |
Unborn Children Clause |
Select this check box if distribution of the proceeds of this employee's life insurance plan is affected by the Unborn Children Clause. |

Beneficiary Name |
This non-editable field displays the beneficiary's name, as specified on the Manage Employee Dependents/Beneficiaries screen. |
SSN |
This non-editable field displays the nine-digit Social Security Number of this beneficiary, as specified on the Manage Employee Dependents/Beneficiaries screen. |
Gender |
This field displays the code for the beneficiary's sex, depending on the option you selected in the Gender group box on the Manage Employee Dependents/Beneficiaries screen. |
Date of Birth |
This non-editable field displays the beneficiary's date of birth as specified on the Manage Employee Dependents/Beneficiaries screen. |
Relationship |
This non-editable field displays the beneficiary's relationship to this employee, as specified on the Manage Employee Dependents/Beneficiaries screen. |
Percent |
Enter the percent of shares this beneficiary receives in the event of a claim payout. This value must be between 1% and 100%.
