This subtask displays the supplemental accidental death and disability insurance plans that you can opt to elect.

Coverage |
This field displays your current level of coverage (for example, "Employee," "Employee + 1," "Family,"). |
Coverage Amount |
This field displays the amount of coverage of the insurance. |
<Frequency> (Negative indicates "Cash Back") |
This field displays your current premium amount, based on the (current) pay frequency ("Monthly," "Semi-Monthly," "Bi-Weekly," or "Weekly"). |
Plan Information |
This field displays the plan information if available. If the plan information is not available, “N/A” displays in the field. |
Start Date | This field displays the start date of the plan. |
Evidence of Insurability |
If applicable, this field displays if the coverage option requires Evidence of Insurability (EOI). Valid values are:

EOI Form Location | If applicable, this field displays the location where the employee can download or find a copy of the EOI form to fill out. |
